This program is for individuals with disabilities who live in families who do not qualify for Texas Medicaid because the family income is above the Medicaid threshold.  Families “buy in” to the program by making monthly payments. Adults with disabilities must work and live in the community (not in a state facility).


Home- and community-based services to individuals with intellectual disabilities who are enrolled in Medicaid and/or receive SSI and are not enrolled in another Texas Medicaid waiver program. There is no age restriction for this program.

Services and supports for individuals with intellectual disabilities who live with their families, independently, or in another community setting who are eligible for Medicaid and are not enrolled in another Texas Medicaid waiver program. There is no age restriction for this program.


Services and supports, including case management, to families caring for an individual with disabilities under age 21 years in the home.

Services and supports to individuals with intellectual and developmental disabilities who live in their family’s home or live independently. There is no age restriction for this program.


Acute and long-term care services and supports for adults age 21 years and older who meet criteria for nursing facility care, to allow them to continue to live in the community. Adults enrolled in this program cannot be enrolled in another Texas Medicaid waiver program and must be have Medicaid. This program is built on a managed care model.
